Case Manager
Location: Eureka, CA
Unit Type: Case Management
Shift: 8 hr day (08:00–16:30)
Schedule: Call required; float to meet department needs; weekend coverage every other or every third weekend
Scrubs: Business casual provided

Unit Overview

• Acute Hospital Case Management experience within the last year is a HARDSTOP
• Must have experience with HMOs, IPAs, advance directives, POLST, and protective services processes
• Epic documentation required
• Travel experience required; first-time travelers can be screened with manager approval

Required

• Certifications: BLS
• Experience: Minimum 2 years acute hospital case management performing care coordination and discharge planning
• Skills:
  • Admission criteria, appeals, audits
  • Concurrent and retrospective review
  • Pre-cert and prior authorization
  • Determine medical necessity per evidence-based guidelines
  • Disease management, plan of care
  • Utilization of InterQual and Milliman Guidelines

Preferred

• Previous charge CM experience
• Teaching or community hospital experience
• LTAC experience

Patient Ratio

• 1:20

Orientation

• 2 full days for Epic/Ministry essentials
• 3 days with preceptor
