Position: Physical Therapist – Home Health
Start Date: 9/29
Contract Length: 13 weeks
Guaranteed Hours: 38 hours/week (flexes up to 40)
Shift: 8-hour variable day shifts, Monday–Friday

Join a collaborative home health team serving adult and geriatric populations with a variety of orthopedic, neurological, and functional mobility diagnoses. This position offers structured autonomy, professional growth, and mileage reimbursement for regional travel.

Job Responsibilities:

• Evaluate, treat, and develop care plans per Physical Therapy standards
• Provide direct care in patients' homes with point-of-service documentation
• Serve a primarily adult/geriatric population with complex rehab needs
• Maintain productivity expectations of 4.5 visits/day
• Travel as needed within the central/northern region (mileage reimbursed)
• Document using NetSmart EMR and company-issued laptop

Requirements:

• Active Physical Therapy license in the state of Wisconsin
• AHA BLS certification (required)
• Valid driver’s license and proof of auto insurance
• Bachelor’s or Master’s degree in Physical Therapy
• Must have (or be willing to obtain) an NPI number
• Previous home health experience required

Additional Details:

• No on-call required
• Weekend requirement: every 6–8 weeks (weekday off provided that week)
• No holidays required
• Scrub colors: any or business casual
• Contract includes up to 36 hours of unbillable low census protection
• GSA hours not guaranteed during orientation week

Home Health experience required.Must be comfortable with adult/geriatric caseloads and home-based care.Prior NetSmart EMR experience preferred.Ideal candidate is self-motivated, adaptable, and communicates well with interdisciplinary teams.Mileage reimbursed at government-approved rate.NPI required (can apply after offer).
