Physical Therapist (Home Health)
📍 Location: Baltimore City / Baltimore County, MD
🕓 Schedule: 30 units per week
🚗 Territory: Assigned nearby areas; mileage reimbursed

Overview:
We’re seeking an experienced and compassionate Home Health Physical Therapist to join our team. The PT will perform evaluations and treatments in patients’ homes, develop individualized care plans, and help patients regain independence and improve mobility.

Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop measurable treatment goals.
• Provide skilled interventions based on physician orders/referrals.
• Review and update care plans regularly according to patient progress.
• Communicate effectively with physicians, caregivers, and interdisciplinary team members.
• Maintain accurate and timely documentation in compliance with company and state standards.
• Supervise PTAs and participate in ongoing professional development.

Requirements:

• Active Maryland Physical Therapist License
• Home Health Care experience required
• OASIS proficiency required
• Reliable transportation and valid driver’s license

Preferred:

• Experience using HCHB (HomeCare HomeBase) or similar Point-of-Care systems
