Description: This position is responsible for directing and supervising all operations of
Hospice and Home Health. Positions assigned to this band interpret and carry out the program or objectives set forth
by a Senior Leader and have substantial authority and independence for decisions related to allocating resources,
staff oversight, budget development, department policy and procedures, and the administrative and financial
success of their assigned areas.
Position Summary Position Summary describes the overall purpose of this position. Using the space below provide a few sentences

Assesses, plans, implements, and evaluates the delivery of patient care and all staff activities for the Hospice &
Home Health units including direct patient care. Maximizes resource utilization and achieves organizational
objectives through continuous improvement and process evaluation for cost effective care and quality clinical
outcomes. Ensures compliance with established organizational, professional and accreditation standards, and
state/federal laws and regulations. In programs with coordinators in place, supports those leaders in addressing
personnel, patient, and operational issues that escalate. Manages patient care operations. Directs and coaches
staff in personnel and patient care processes.
Education: Graduation from an accredited Nursing program.
Preferred Education: Bachelors degree or higher.
Experience(s): Five years of current nursing experience. Two years of supervision/management experience.
Note: Can run concurrently.
License(s): Current State of Alaska Registered Nursing License
Certification(s): Current Basic Life Support (BLS
Leadership/Supervision: • Plans and supervises the Home Health and Hospice program(s). • Directly oversees the interviewing, hiring, retention, evaluating and counseling/terminating (with Bartlett Regional Hospital Human Resources) of supervised staff. • Plans the departmental orientation of new employees and validates personnel completion of
orientation in a timely manner. • Ensures that established objectives are implemented by supervised staff. • Provides individual or group support concerning job related stress or issues for supervised
staff. • Serves as a role model for all colleagues by setting an example of high standards in dress,
conduct, cooperation, dependability, and job performance. • Oversees staffing and assignments. • Oversees and manages the 24 hours/day, 7 days/week on-call coverage schedule and
rotation. • Supervises the planning, coordination, and delivery of the interdisciplinary group
interventions. • Assigns patient/family to appropriate staff member(s). • Supervises the maintenance of medical records, assuring accuracy completeness and
compliance with licensing regulations, certification standards, legal and ethical standards. • Monitors equipment for appropriate use. • Oversees or arranges for clinical supervision of assigned staff. • Conducts appropriate supervisory visits which allow for clinical observation of performance
of assigned team members.
Home Health and Hospice Operations/Administration: • Works with the Home Care Services Administrator and Bartlett Regional Hospitals Senior
Leadership Team to identify budgetary requirements and determine appropriate use of
allocations. • Prepares and maintains current policies and procedures that meet Medicare, Medicaid,
accreditation standards, state, and federal laws, etc. and implements; revises concurrently. • Supports and carries out existing policies and procedures and uses appropriate channels for
change of such policies. • Participates in developing standards that ensure safe and therapeutically effective service to
patients and families. Has joint responsibility with the Administrator for seeing that
standards are met. • Participates in developing objectives for Home Health & Hospice services. • Participates in establishing the functions and qualifications for each service position.
coordinates interdisciplinary team services. • Consults with the Administrator to determine a staffing pattern which will accomplish stated
objectives and promote maximum level of utilization of personnel. • Organizes Home Health & Hospice to delineate and delegate authority, functional
responsibility, lines of relationship and communication to provide safe and therapeutically
effective service. • Participates in coordinating Hospices services with services of other community agencies. • Develops a cooperative relationship and communicates effectively and professionally with
physicians. • Assures physician approval of plans for continuity of medical care. • Assures ongoing assessment of patient/family needs and implementation of interdisciplinary
group plan of care. • Ensures that patients plans of care are developed, implemented, and evaluated. • Ensures compliance with federal, state, and local regulations, accreditation standards, and
Home Health & Hospice policies in all patient care aspects of the Home Health and Hospice
program(s). • Addresses and corrects all patient concerns. Investigates and reports any problem relating to
patient care or conditions which might harm the patient and/or employee well-being per
Bartlett Regional Hospital policy. • Immediately reports any accident, incident, lost articles, or unusual occurrence to the
Administrator and per Bartlett Regional Hospital policy. • Oversees Home Health and Hospices ongoing Quality Assurance/Performance Improvement (QAPI) program in conjunction with Bartlett Regional Hospitals QAPI program. • Oversees the Home Health and the Hospices Emergency Management / Communications
Plan in conjunction with Bartlett Regional Hospitals Emergency Management program.
Direct Patient Care: • Provides Home Health and/or Hospice nursing services, treatments, and preventive
procedures when needed/as necessary. • Participates in on-call system and is responsible for providing on-call coverage when
needed/as necessary. • Regularly assess and reassess the nursing needs of the Hospice patient. • Conducts patient case conferences, in-services, staff meetings and maintains
documentation; participates in community education projects.
Other Duties: • Observes confidentiality and safeguards all patient related information. • Ensures that all necessary supplies and equipment are available. • Attends pertinent continuing education programs other than routine in-services and shares
information with staff. • Serves as a resource person to employees. • Other duties as assigned
