Registered Nurse (RN), Home Health

Position Title: Registered Nurse – Home Health
Location: 320 Alpenglow Lane, Livingston, MT 59047
Assignment Type: Contract
Contract Length: 13 weeks
Schedule: 40 hours/week | Preferred Monday–Friday, 8-hour days (5×8)
Start Date: ASAP
Call Requirement: 2–4 weekday nights per week
Weekend Requirement: 1–3 weekends over the course of the contract
Holiday Coverage: Ability to work Thanksgiving and/or Christmas strongly preferred

Position Summary

Livingston Healthcare is seeking an experienced Home Health Registered Nurse to join its dedicated community-based care team. The Home Health RN will provide skilled nursing services to patients in their homes, supporting recovery, independence, and quality of life. This role requires strong clinical judgment, independence, and the ability to adapt to patients’ diverse needs in a home setting.

Key Responsibilities

• Deliver safe, effective, and compassionate nursing care to patients in their homes.
• Conduct patient assessments, develop individualized care plans, and evaluate progress.
• Administer medications, IV therapies, wound care, and other skilled nursing interventions.
• Educate patients and families on treatment, medications, and self-care techniques.
• Coordinate with physicians, interdisciplinary teams, and community resources.
• Provide occasional inpatient support on the Med/Surg floor as needed.
• Maintain accurate, timely, and thorough documentation of care.

Requirements

• Active RN License (Montana or Compact State).
• Experience:
  • Minimum 2–3 years of Home Health or related nursing experience.
  • Hospice experience preferred (training provided if needed).
• Certifications: BLS and ACLS required.
• Strong assessment, case management, and patient education skills.
• Ability to manage a caseload of:
  • 4–5 patients per day (8-hour shifts).
  • 5–7 patients per day (10-hour shifts).
• Reliable transportation is required for community-based visits.
