Title: RN/Case Manager

Location: Santa Fe, NM
Shift: 8:00 AM – 4:00 PM
Duration: 32 Weeks
 

Responsibilities:

• Coordinate and manage patient care plans across the continuum of care.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure quality patient outcomes.
• Provide education and support to patients and families regarding discharge and post-hospital care.
• Monitor patient progress and adjust care plans as needed.
• Ensure compliance with hospital policies, procedures, and regulatory requirements.
• Participate in discharge planning and transition-of-care coordination

Requirements:

• Active RN license (New Mexico or Compact State).
• Minimum 3 years of experience as a Registered Nurse Case Management or related specialty.
• Discharge planning experience required.
• Strong communication, organizational, and critical thinking skills.
