Job Description

American Traveler seeks an experienced RN Care Manager with trauma experience and an active NH or compact RN license for an inpatient care management role.

Responsibilities

• Work in inpatient units within a hospital setting
• Day shift schedule, Monday through Friday from 8:00 am to 4:40 pm
• No weekend or on-call responsibilities
• Responsible for managing a daily caseload of patients with diverse care needs
• Utilizes Epic EMR for patient documentation and order entry
• No local candidates; candidates must live outside a 75-mile radius
• Oversee patient caseloads, including conducting initial assessments and attending interdisciplinary rounds
• Develop and manage patient care plans in collaboration with the healthcare team, patients, and families
• Coordinate post-discharge needs such as home health, inpatient rehab, DME, and nutritional orders
• Regularly communicate with medical teams and patients regarding care progress and discharge planning
• Interacts with post-acute care referral sources to facilitate continued patient care

Requirements

• Active NH or compact RN license required
• Certified care manager credential required
• Bachelor of Science in Nursing (BSN) required
• Minimum of 3 years inpatient clinical experience required
• At least 1 year of care management experience required
• Recent Level 1 or comparable Level 2 trauma experience within the last 2 years required
• Current BLS certification required
