About The Position
Job Title: RN Case Management/Discharge Planning Profession: Registered Nurse Specialty: Case Management (Utilization Review) Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 2 years of Acute Care Management experience in discharge planning License: RN License required Certifications: BLS, RN Case Manager certification preferred, CCM preferred but not required Must-Have: - RN Case Manager with at least 2 years of Acute Care Management experience in discharge planning - Experience must include at least 6 months at a large academic Medical Center hospital working with various inpatient units for medical discharge - Experience with EPIC charting Description: This position involves case management and discharge planning. You will be responsible for covering all inpatient units throughout the facility. The average caseload will be 15-20 patients per day. You will collaborate with a team of social workers, which consists of full-time and part-time professionals. Common duties include managing medical discharges, with some social discharges possible. There are contracts with local facilities to assist with complex patient placements, adhering to specific criteria. This facility serves a variety of complex patients, as the only trauma center providing essential services. You will report directly to the Social Work Manager for coordination and oversight. Shifts are Monday through Friday, with a standard schedule of 8:00 AM to 4:30 PM. Weekend and holiday coverage will be rotated as needed. You may consult with respiratory therapy and pharmacy as necessary during the discharge planning process. Department-specific orientation will be provided, including general facility orientation. You will work alongside the case management team to ensure support and collaboration in your role. Additional questions may arise regarding problem-solving in complex patient placement scenarios.
