Job Title: Integrated Case Manager/ LMSW
Location: Detroit, MI – 48202
Shift Hours: Days 8:00 AM – 4:30 PM, rotating weekends          HPW: 40
Contract Length: 13 Weeks

Job Description:

This position requires highly experienced candidates with acute inpatient case management and discharge planning background. Training on unit will be minimal—candidates must be able to pick up quickly and work efficiently.

Requirements:

• Minimum 5 years of acute inpatient case management experience (does not include telephonic, utilization management, or therapy experience)
• Strong computer skills
• Epic EMR experience
• Nursing degree or MSW
• BLS certification through AHA
• Current valid MI RN licensure or MI LMSW licensure
• Acute discharge planning experience
• Recent (within the last two years) acute inpatient case management experience

Additional Details:

• Typical case ratio: 1:21–25 (will not meet with each patient; determined based on individual risk factors)
