Schedule: 40 hrs/week, 8-hr shifts Days 8 hours shifts, 8a-4:30p

Start: 09/23/2025 | End: 01/03/2026 | Long-term contract (holiday coverage)
 

Overview:
We are seeking an experienced RN Case Manager to support patient care coordination at Henry Ford Allegiance Health. The RN will manage a designated patient caseload, ensuring quality outcomes, timely discharge planning, efficient resource use, and accurate documentation for reimbursement.

Responsibilities:

• Coordinate care and discharge planning across the continuum.
• Facilitate payor authorization and precertification processes.
• Collaborate with physicians, payors, and interdisciplinary teams to remove barriers to care.
• Support clinical documentation efforts and serve as a resource for coding accuracy.
• Apply process improvement methods to evaluate and improve patient outcomes.
• Educate patients and families regarding care plans and available resources.

Requirements:

• Active Michigan RN License
• BSN degree required
• Minimum 2 years of Case Management experience (utilization review only does not qualify)
• Strong communication, organization, and negotiation skills

Preferred:

• 3+ years RN clinical experience
• Experience with Epic and Care Manager systems
• Knowledge of discharge planning and managed care reimbursement

Days 8 hours shifts, 8a-4:30p
