Position: Registered Nurse (RN) – Case Manager (Home Health/Hospice)
Location: Fond du Lac, WI and surrounding counties
Start Date: ASAP
Contract Length: 13 weeks (with potential to extend)
Guaranteed Hours: 40 hours/week
Shift: Monday–Friday, 8:00 AM – 4:30 PM
Weekend/On-Call: None

Serve as a Case Manager supporting home health and hospice patients across Fond du Lac, Dodge, and Columbia counties. This is a field-based role, requiring travel between patient homes and occasional facility visits. The RN will perform comprehensive assessments, coordinate care plans, collaborate with interdisciplinary teams, and manage end-of-life symptom support for patients and families.

🧠 Responsibilities & Unit Overview

• Perform 5 patient visits per day in a home or facility setting
• Conduct comprehensive assessments and care planning
• Manage care transitions and coordinate with MDs, SWs, chaplains, CNAs, and facility staff
• Educate patients and families on symptom management and end-of-life signs
• Manage wound care, pain control, and medication education
• Maintain accurate and timely documentation in the EMR
• Engage with families and patients to ensure compliance with care plans
• Act as patient advocate, ensuring privacy and continuity of care

✅ Requirements

• Active RN license in the state of Wisconsin
• AHA BLS certification
• Valid driver’s license, reliable vehicle, and current car insurance
• Strong wound care experience required
• Must be comfortable working independently in a field-based role
• Experience with frail elderly and end-of-life care strongly preferred
• Minimum 2 years of Home Health/Hospice experience preferred
• Must be willing to complete skills checklist and provide proof of recent compliance training
