Registered Nurse – Case Manager – Day Shift
Facility: LifeBridge Health – Sinai Hospital
Location: 2401 W Belvedere Ave, Baltimore, MD 21215
Start Date: September 15, 2025
Duration: 14 weeks
Shift: Day Shift (8:00 AM – 4:30 PM)
Schedule: Monday–Friday, 2 weekend day shifts/month
Guaranteed Hours: Full-time
Unit: Case Management

Position Overview:
Seeking an experienced RN Case Manager with at least 2 years of acute care case management experience. The role involves discharge planning and managing a patient ratio of approximately 1:30 in a high-volume hospital setting (300+ beds). The patient population covers all age groups and diverse diagnoses including trauma, neuro, cardiac, med/surg, open heart, behavioral health, and bone lengthening.

Certifications & Requirements:

Active Maryland or Compact RN license required
BLS required
Minimum 2 years acute care case management experience
No first-time travelers

Skills & Competencies:

Proficient in discharge planning and patient case management
Comfortable working in a fast-paced acute hospital environment
Experience with Cerner EMR preferred
Ability to float within the facility as needed

Facility Details:

400 daily patient census
Interdisciplinary support including social services and interpretation services
Observes major holidays (New Year’s Day, Independence Day, Thanksgiving, Christmas)
Flu vaccine required (medical exemptions allowed)
COVID-19 vaccine not required
