Shift: Days, 5x8 (8:00 AM – 4:30 PM)

Job Requirements

Minimum 2 years acute care case management and discharge planning (required)
Prior travel nursing experience required (no first-time travelers)
Active RN license
Certifications: BLS (required)
Charting: Epic (required)
Patient ratios: Days 1:25; Weekends 1:45
Must have hospital experience (behavioral health experience alone does not qualify)

Facility & Unit Details

Unit: Case Management (Acute Care Hospital)
Caseload: Up to 25 patients daily
Orientation: 40 hours
Weekend rotation required (minimum 4 weekend shifts per 6-week schedule)
Patient Age Groups: Neonates, Adolescents, Adults, Geriatrics
Interdisciplinary Support: Physical Therapy, Social Services

Skills Needed

Assessments, care coordination, discharge planning, utilization review
Organize and participate in family meetings
Arrange DME, SNF, and ambulance transport independently
Experience with HMOs, IPAs, adoption/foster care, substance abuse treatment, homelessness, advance directives, POLST, and protective services
Knowledge of CMS, Joint Commission, DRGs, and regulatory standards
