Shift/Schedule: Evening shifts 
Guaranteed Hours: 40 hours/week

• Job Description: -
• Provide direct patient care in a long-term care setting under the supervision of an RN or physician.
• Administer prescribed medications and treatments accurately and on time.
• Monitor and document patient conditions, reporting changes to the supervising nurse/physician.
• Assist with daily living activities, ensuring resident dignity and quality of life.
• Maintain accurate medical records and ensure compliance with facility policies and state regulations.
• Collaborate with the healthcare team to develop and implement care plans.
• Ensure infection control and safety protocols are strictly followed.
  
  Required Skills & Experience:
• Active New York LPN License
• Minimum 2 years of recent LTC experience
• BCLS (Basic Cardiac Life Support) Certification
