Job Description:

Supplemental Health Care is seeking LPNs in Portage, Michigan with prior Medicare Certified Visits experience for contract assignments with a partnering home health care provider. Local or travel, we want to connect you with assignments that fuel the miracles within your life.

Home Health Nurse Contract Details:

• $1,380 – $1,526 per week*
• 13-week contract
• SHC Home Health Nurses are paid by the hour, not by the visit
• LPN will have a designated territory of coverage
• Average weekly expectation is 30 units of productivity
• Home Health contracts with SHC require previous experience with Medicare Certified Agency

Requirements:

• Medicare Certified Agency Experience
• Electronic Charting experience
• Minimum of 1 year recent home health nursing experience (private duty does not qualify)
• Current Michigan Nursing License
• Valid U.S. Driver’s License
