Candidates who are interested in accepting a permanent role with this facility after a 13 week contract are encouraged to apply.**WA LICSW, WA LSWAIC, OR WA LASW required upon submittal, temporary licensure is accepted**Open to true travelers as well as locals!First time travelers are accepted.This is a FLOATING social work position supporting 2 Adult Day Care Programs in Seattle Area. **Will float between 4515 Martin Luther King Jr Way S, Suite 100, Seattle WA 98108 & th Ave SW Seattle, WA 98126**As a member of the interdisciplinary team, a candidate in this role provides social work services to participants in a manner which is consistent with the mission and core values of the Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.A candidate in this position will do some, though not all, of their assessments in the homes of the participants. It is a moderate to light amount of in-field work, and it's definitely far less than a home health or hospice social worker would do. This could be within the radius of Edmonds to Auburn.A candidate will be reimbursed for mileage and hours driving from one work location to another. This will count as paid time worked (ex: MSW drives from PACE center to participant's home for assessment, then drives back to PACE center). It is expected that all travel will occur within the workday of 8am-5pm, which would all be paid time. Mileage will be reimbursed when the candidate is traveling from one work location to another, but the candidates commute from their personal home will not be paid. Special Procedures/Unit Details : This position requires both a masters in social work, AND one of the following:a Washington Clinical Independent Social Worker Associate License, Washington Clinical Independent Social Worker Associate Temporary License, Washington Clinical Independent Social Worker License, or Washington Clinical Independent Social Worker License Temporary License.2 Years of experience in social work with geriatric age group required. Special Equipment : Home Visits will be required for some of our participants. Social worker will start from the center and leave to the community to do those visits.All the social workers are assigned a laptop and cell phone to perform their duties. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency. For candidates on 48-hour schedules, OT after 48 hours in a week Modules: Modules are non-billable, average 16 hours spent.
