About The Position

Roles & Responsibilities - LCSW

• Provide counseling, crisis intervention, and psychotherapy to individuals, families, and groups.

• Conduct psychosocial assessments and develop individualized care plans.

• Collaborate with physicians, nurses, and interdisciplinary teams to address patient needs.

• Support patients dealing with mental health issues, chronic illness, trauma, grief, or life transitions.

• Assist patients in accessing community resources, financial aid, housing, or rehabilitation programs.

• Maintain accurate and confidential patient records in compliance with HIPAA and organizational standards.

• Participate in discharge planning, ensuring continuity of care after hospitalization or treatment.

• Provide education on coping strategies, stress management, and mental health awareness.

• Adhere to state licensing requirements and professional code of ethics.
