Job Description

American Traveler is seeking an experienced RN Hospice Case Manager with hospice home care and current VT or compact RN licensure.

Responsibilities

• Provide hospice case management services in a home health setting
• Patient population consists of hospice clients in community and rural locations
• Position follows a Sunday through Thursday schedule, with M-F orientation in the first week
• Use of McKesson/Netsmart EMR is preferred
• Responsible for managing hospice patient caseloads and coordinating interdisciplinary care
• First week includes orientation Monday through Friday
• Candidates should ideally not request time off for this contract
• Local candidates are accepted

Requirements

• Active VT or compact state RN license required
• Minimum of 1 year hospice case management experience
• Prior hospice care experience in a home setting is required
• Current BLS certification required
• Experience with McKesson/Netsmart EMR preferred
• Rural travel experience preferred
