Job Description

American Traveler is seeking a Home Health Physical Therapist with prior home health and adult/geriatric experience for daytime shifts in Wisconsin.

Responsibilities

• Work in a home health setting visiting adult and geriatric patients
• Caseload includes orthopedic, neurological, and functional mobility diagnoses
• Average productivity expectation is 4.5 patient visits per day
• Variable day shifts Monday through Friday, with rotation to work one Saturday every 6-8 weeks
• Position requires occasional travel to central and northern regions
• Mileage reimbursed at government-approved rates
• Uses NetSmart EMR and laptop for point of service documentation
• Cell phone provided for GPS and communication
• Primary responsibilities include evaluating, planning, and delivering patient care in accordance with Physical Therapy standards
• Candidates should be resilient, adaptable, and skilled in managing change
• Social, hard-working individuals are preferred
• Scrub colors are flexible, or business casual attire may be worn
• No holiday coverage required
• PTs are given a day off during the week when scheduled for Saturday rotation

Requirements

• Bachelor’s or Master’s Degree in Physical Therapy required
• Current AHA BLS certification required
• Active Wisconsin Physical Therapy license required—license required if applicable
• Valid driver’s license with proof of car insurance required
• Must have or be able to obtain an NPI number
• Previous home health experience required
• Experience working with adult and geriatric populations in a variety of diagnoses required
• Ability to document to meet regulatory and reimbursement requirements
