Job Description

American Traveler is seeking an experienced Physical Therapist for a home health assignment with a Monday to Friday schedule.

Responsibilities

• Home health setting providing physical therapy services in patient residences
• Regular schedule of Monday through Friday, 8:00am to 4:00pm
• Patient population includes individuals requiring rehabilitation therapy and care management
• May carry out procedures such as PT/INR monitoring, blood pressure checks, staple or suture removal, and wound care
• Responsible for OASIS assessments and documentation in accordance with agency, state, and federal guidelines
• Responsible for monitoring and adjusting daily patient schedules to maximize productivity and address patient needs
• Develops individualized plans of care and documents clinical interventions and goals
• Provides skilled physical therapy treatment and patient/caregiver education in the home setting
• Coordinates care with a multidisciplinary team including nurses, physicians, and other rehab professionals
• Supervises and directs the activities of PTAs, PT students, and home care aides as needed
• Ensures timely and accurate completion of documentation and billing forms
• Participates in in-service programs, marketing, and community health initiatives
• Works collaboratively with quality improvement staff and follows agency protocols for documentation and patient care processes
• Facilitates safe patient discharges and transfers in compliance with regulations and agency policies
• Contributes to peer review audits and maintains professional development

Requirements

• Minimum 2 years of recent physical therapy experience required
• License required if applicable
• Must meet state requirements at time of acceptance
