Join a dedicated home health care team serving patients throughout Washington County, MD. This role focuses on in-home therapy, OASIS assessments, and oversight of PTAs, providing a unique opportunity to impact patient recovery and independence in the comfort of their homes.

🧠 Responsibilities & Role Overview

• Perform physical therapy evaluations and implement treatment plans in accordance with physician orders
• Conduct OASIS assessments and complete documentation accurately and on time
• Reassess patients and modify goals or interventions as appropriate
• Provide in-home therapy visits to adult and geriatric populations
• Oversee and support Physical Therapist Assistants within assigned territory
• Recommend and assist with adaptive equipment as needed
• Collaborate with medical providers, caregivers, and interdisciplinary teams to ensure coordinated care
• Maintain compliance with HIPAA, state regulations, and agency policies
• Participate in staff development and continuing education as required

✅ Requirements

• Active Physical Therapy license in the state of Maryland
• Recent home health experience strongly preferred
• OASIS documentation experience required
• HomeCare HomeBase (HCHB) EMR experience preferred
• AHA BLS certification
• Strong interpersonal, time management, and documentation skills
• Must be comfortable working independently in a field-based role
