Physical Therapist – Home Health

Schedule: Full-Time, Monday–Friday, 8-hour shifts
Hours: 40 hours per week
Workload: Approximately 30 units per week
Location: Baltimore, MD area (city and surrounding counties)

Position Overview

We are seeking a licensed Physical Therapist to join a home health team providing patient-centered care in the community. The ideal candidate will have experience in home health and strong clinical skills, with a passion for helping patients maximize independence and quality of life.

Responsibilities

• Perform physical therapy evaluations and treatments based on physician referrals.

• Complete comprehensive assessments using appropriate tools and procedures.

• Review patient records and histories to ensure safe and effective care planning.

• Establish measurable goals and individualized treatment plans to support patient independence and function.

• Reassess progress regularly and adjust treatment plans as needed.

• Collaborate with physicians, care teams, and family members to ensure coordinated care.

• Educate patients and caregivers on exercises, functional mobility, and use of adaptive equipment.

• Document all evaluations, treatments, and progress in accordance with regulatory and organizational standards.

• Supervise Physical Therapist Assistants and Aides as required by state practice guidelines.

• Participate in staff development, team meetings, and continuing education opportunities.

Requirements

• Current, active Physical Therapist license in the state of Maryland.

• Prior home health experience required.

• Knowledge of OASIS documentation required.

• Valid driver’s license and reliable transportation.

Preferred Qualifications

• Experience with HCHB (Home Care Home Base) or similar electronic medical record systems.

• Strong communication skills and ability to work independently in the field.

• Experience supporting diverse patient populations, including elderly and medically fragile individuals.
