Job Description

American Traveler is hiring an experienced RN Case Manager in California with required home health case management experience and Epic charting knowledge preferred.

Responsibilities

• Work in a home health case management setting
• Day shifts, Monday through Friday, from 8:00 AM to 4:30 PM
• Manages a designated patient caseload in accordance with established standards and regulations
• Epic charting experience is preferred
• Coordinate interdisciplinary team for patient care planning and service implementation
• Responsible for management of visit utilization and expense control for assigned patients
• May assist in orientation, competency evaluation, and consultations related to area of expertise
• Modules are completed during orientation via e-learning and are non-billable up to 16 hours
• Only one holiday request for time off will be approved
• Must reside at least 50 miles from the facility to qualify for travel rate
• Returning travelers must have a minimum one-year break before reapplying

Requirements

• Active CA RN license required at time of consideration
• Current BLS certification from AHA or American Red Cross required
• Minimum of 2 years of home health case manager experience required
• Must be able to provide two supervisor references obtained within the last year
• Must be available to complete lab work within 3-5 days of contract signing
