RN Case Manager

Job Location: 500 Upper Chesapeake Dr Bel Air, MD 21014-4324

Shift: Day 5x8-Hour (08:00 - 16:30)

Description:

Unit/Details: 5x8hr days either 08:00 or 08:30 start depending on needs. Holidays as needed.
Average Daily Census – 15 cases daily

Patient Diagnosis - INPT case management providing and coordinating medical home care, placement, & social issues-wrap around type services for safe discharges.
2 yrs minimum experience. Will take a first time traveler. Must have inpatient experience and preferred EPIC.
