Registered Nurse – Case Manager – LBH - Sinai Hospital

Location: 2401 W Belvedere Ave, Baltimore, MD 21215
Unit: Case Management (1000-6620 CM)
Duration: 14 weeks
Positions: 1
Shift: Day 5x8-Hour (8:00 AM – 4:30 PM)
Weekend Coverage: 2 weekend day shifts per month
Scheduling: Block schedules (monthly schedule from 1st to last day)

Job Overview:

Manage acute care case management for a high-volume hospital (300+ beds). Responsible for discharge planning and coordination for diverse patient populations across multiple specialties and age groups.

Experience & Certifications:

• Required: Minimum 2 years acute care case management experience
• No first-time travelers accepted
• Certifications Required:
  • BLS
• Charting System: Cerner preferred
• License: Valid Maryland or Compact license required at time of submission
• References: Required

Patient Ratio:

• Approximately 1:30

Patient Population & Diagnoses:

• All age groups including neonates, infants, pediatrics, adolescents, adults, geriatrics
• Patient diagnoses include trauma, neuro, cardiac, med/surg, open heart, behavioral health, bone lengthening

Work Setting & Support:

• Acute hospital environment
• Interdisciplinary support includes interpretation services, social services, unit secretary

Hard Stops:Must have 2 years acute care case management experienceNo first-time travelers accepted
