Physical Therapist - Home Health - (PT - HH)
StartDate: 10/27/2025 Available Shifts: 8 D Pay Rate: $ - $
No more than a 30-day work gap and MUST HAVE references within the past year, no exceptions! PT with 1+ years of experience needed for Home Health - previous home health experience is required. ND license and BLS required. Must have current Driver's License & Auto Insurance. Area covered is 60mi radius around Valley City, ND, can expect 5-7 patients per day. EMR is HomeCare/HomeBase. Dress is CSH color scrub or Business Casual. All RTO is required at time of submission or will be denied.

Licenses: PT(Compact Accepted)
Certifications: BLS-AHA
SSN Required
DOB Required
Work History: Should not be more than 30 days of gap in entire work history
References: 1 Reference in the last 12 months
Must have current Driver's License & Auto Insurance
