• Description: Summary: General orthopedic surgeon needed for a 3-6 month assignment in Prescott Arizona to cover gaps of care prior to new providers onboarding.RATE CAPS FOR SUBMISSION:Malpractice Must be included in rates. Orientation 225 - half the hourly/callback rate kicks in after 8 hours. Per hour oforientation or training beyond 8 hours Hourly 450 - Per hour not OT Holiday 675 - Must be 1.5x the hourly rate. Per holiday hour no OT Nightly Call 1550 - Per night call to hold the pager to include 2.5 hours gratis Holiday Nightly Call - 2325 - Per night call to hold the pager to include 2.5 hoursgratis On Call Daily 3100 - Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - 4650 - Per 24 hours to include 4 hours gratisCall Back 450 Per hour ofpatient contact hours after gratis hours are completed gratis prorate to MSAterms.
