Clinical Nurse Coordinator

Provide clinical focus on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team.

Provides advanced clinical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care.

Provides focused assessment to evaluate patients and their support systems for real and potential health and safety issues, educational needs, and resource availability upon initial interaction, throughout care and/or treatment, and at times of transition in collaboration with the multidisciplinary team.

Facilitates patient access and progression through the Valve Center.

Provide education to staff, patients and families as needs are identified. Provides clinical and administrative program support to the surgeon(s), cardiologist(s), and Advanced Practice Clinicians [APC(s)] associated with the Structural Heart program, industry sponsors as well as staff and patients.

Maintains communication with referring clinicians.

Assists the Clinical Research Coordinators [CRC(s)] for Structural Heart related research trials.
