Position Summary

Sinai Hospital is seeking a compassionate, dedicated, and licensed Social Worker to join our palliative care team. This full-time position provides support at Sinai Hospital in Baltimore, MD. The Social Worker plays a vital role in supporting patients and families through emotional, social, and practical challenges related to critical illnesses. 

Key Responsibilities

• Assess patient and family needs promptly upon admission, with attention to emotional, environmental, and financial concerns.

• Collaborate with physicians, nurses, and other team members to develop individualized care plans.

• Identify and refer patients to appropriate community resources based on assessed needs.

• Support patients and families in understanding and navigating health decisions, advance directives and placement options.

• Participate in interdisciplinary team meetings and document social work interventions and progress.

• Maintain accurate and timely documentation in accordance with professional, regulatory, and internal standards.

• Monitor and evaluate patient condition and progress, communicating changes to the care team.

• Uphold patient confidentiality and promote a team-based, ethical approach to care.

• Engage in ongoing professional development, licensure maintenance, and continuing education.

Qualifications

• Must hold an active LCSW-C, LCSW, or LMSW license  endorsed in the state of Maryland .

• Strong interpersonal, communication, and problem-solving skills.

• Ability to work independently and as part of a multidisciplinary team.
