Position Summary

The Social Worker (MSW) delivers direct social work and care management services to PACE participants, focusing on comprehensive support to enhance their quality of life and independence in the community. Responsibilities encompass participant screening, case management, counseling, referrals, and the planning and implementation of social work services tailored to participants and their families.

As a liaison among the interdisciplinary team, caregivers, and community agencies, the Social Worker ensures that participant needs are effectively addressed, and services are delivered seamlessly. This role is integral in promoting participant well-being, advocating for their needs, and supporting the goals of the PACE program.

Key Responsibilities:

• Engage in initial assessments, care planning, and periodic reassessments of participant cases, ensuring completion before team meetings.
• Accurately document participant changes and updates in the medical records, adhering to industry standards.
• Collaborate effectively with Interdisciplinary Team (IDT) members regarding participant needs and care plans.
• Maintain comprehensive case management records, including periodic reassessments.
• Coordinate 24-hour care delivery and provide counseling for participants and their families.
• Facilitate group counseling sessions and activities aimed at meeting participant needs.
• Refer participants and families to appropriate community resources, acting as a liaison and advocate.
• Encourage effective team relationships to foster collaboration.
• Consult with staff on the social, emotional, and cultural impacts on participant health and available community services.
• Participate in the development of program and policy for social work services and contribute to Quality Improvement processes.
• Conduct home visits to evaluate and support participant needs.
• Actively engage in IDT meetings, contributing to care planning discussions.
• Perform additional duties as assigned.

Experience and Qualifications:

• Master's degree in Social Work from an accredited institution.
• Membership in the Academy of Certified Social Workers (ACSW) or other NASW-recognized certifications preferred.
• Current state licensure required.
• Two years of experience in an interdisciplinary team within a hospital, nursing home, or community-based setting is preferred.
• One year of experience working with frail or elderly populations is preferred; training will be provided if not present.

Pre-Employment Requirements:

• Must have reliable transportation, a valid driver's license, and necessary auto insurance.
• Medical clearance for communicable diseases and up-to-date immunizations required before direct participant contact.
• Pass a thorough criminal background check, including federal and state exclusion lists and reference checks.
