Responsibilities

Provide direct discharge planning services and assistance with psychosocial services for the patient and family.
Collaborate with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient's health care needs.
Utilize broad experience and knowledge of appropriate healthcare resources for patients of all ages.
Promote continuity through communication of the patient's plan of care.
Report to the Clinical Resource Management Coordinator.

Qualifications
Job Requirements

Master of Social Work (MSW)
Clinical emphasis from a CSWE accredited graduate school
Kansas licensure at the LMSW level

Preferred Job Requirements

Two semesters of graduate field experience in healthcare setting for a new graduate or previous experience in a healthcare setting for an experienced graduate
