Role Overview
The Social Worker (MSW) will deliver essential social work and care management services to PACE program participants, supporting their journey towards independent living. This role involves conducting participant screenings, providing case management and counseling, and making referrals, all while implementing social work services that cater to the individual and their families.
As a vital link between the interdisciplinary team, caregivers, and community agencies, the Social Worker will ensure that participants receive the support they require and that their services are coordinated effectively. This position is crucial for advocating on behalf of participants and advancing the goals of the PACE program.
Key Responsibilities:
- Conduct initial assessments and care plans, with regular reassessments (minimum every six months), ensuring timely completion before team meetings.
- Accurately document participant changes and updates in compliance with industry standards.
- Communicate efficiently with the Interdisciplinary Team (IDT) about participant needs and care strategies.
- Maintain comprehensive case management records with periodic reassessments.
- Coordinate 24-hour care delivery and provide counseling to participants and their families.
- Plan and facilitate group counseling and activities tailored to participants’ needs.
- Refer participants to suitable community resources while acting as an advocate.
- Foster collaborative relationships within the team to promote cohesion and effectiveness.
- Advise on the social, emotional, and cultural impacts on participant health and available community services.
- Contribute to social work policy development and engage in quality improvement initiatives.
- Conduct home visits to evaluate participant needs personally.
- Actively participate in IDT meetings contributing to planning and discussions.
- Adhere to the Social Workers' Licensing Act of 1991 and maintain compliance with state regulations.
- Engage in additional duties and special projects as assigned.
Qualifications:
- Master's degree in Social Work from an accredited institution recognized by the Council on Social Work Education.
- Preferred: Membership in the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification.
- Must possess a current license in the required state.
- Two years of experience in interdisciplinary teams preferably in healthcare settings.
- Preferred: One year of experience with frail or elderly populations; training will be provided if necessary.
Pre-Employment Requirements:
- Must have reliable transportation, a valid driver’s license, and meet auto insurance requirements.
- Medically cleared for communicable diseases with up-to-date vaccinations prior to direct participant contact.
- Successful completion of a comprehensive criminal background check, including verification of education, licenses, and references.
