Social Work Manager Elder Care / Community-Based Services

We are seeking a Social Work Manager to lead a team of social workers and behavioral health specialists serving seniors in community-based care programs in San Jose, CA. This role is ideal for a clinical leader who wants to make a meaningful impact on the lives of older adults while supporting a collaborative, interdisciplinary team.

As Social Work Manager, you will oversee social services and behavioral health programs, ensure compliance and quality in care delivery, and foster a positive team culture. This position includes supervision, strategic oversight, and collaboration with internal and external stakeholders.

Responsibilities:

• Supervise Social Work Supervisor, Social Workers, Behavioral Health Specialists, and Social Work Assistants, including hiring, training, coaching, and performance management
• Ensure effectiveness of social work assessments, care plans, and IDT participation
• Oversee delivery of social work interventions aligned with participant needs
• Ensure compliance with documentation, care plan completion, participant rights, and grievance procedures
• Collaborate with behavioral health team to oversee programming, including support groups, individual counseling, and family education
• Participate in end-of-life care coordination and support
• Conduct home visits as necessary

Requirements:

• Masters degree in Social Work (MSW) required
• Social work licensure preferred
• Minimum 2 years supervisory or leadership experience in social work strongly preferred
• Minimum 1 year of experience with frail or elderly populations
• Excellent clinical, organizational, and communication skills
• Valid state drivers license with a clean DMV record
• Recent vehicle insurance coverage required
