Job Summary:
Reports to the Operations Manager, the RN Nurse Navigator functions as a member of the multidisciplinary team of care providers who reflect a culture of cooperation, enthusiasm, and respect. The Navigator is responsible for coordinating the appropriate care of the patient based on an evaluation/assessment of the patient (telephonically or in-person) that matches the patients desires to the most applicable program that the Surgery Department offers. This includes education, continuity of care and support through all phases of diagnosis, treatment, and follow-up care for patients in seeing that maintain a high level of engagement and participation in the program that is critical to their success and the goals that they desire. The role is mainly clinical, but also one that incorporates business development and a high level of patient engagement skills.

Schedule: Monday - Friday, Hours 8am - 4:30pm, In Office
LICENSES AND CERTIFICATIONS:
  • Must have a valid driver's license for travel between office sites.
  • Current registration to practice nursing in the state of Kentucky.
  • Obtains required CEUs, including all state required courses to maintain license.
  • BLS
  • Active Kentucky RN licensure
EDUCATION:
Graduate from an accredited nursing program
YEARS OF EXPERIENCE:
Required: 3+ years of RN experience
DUTIES AND RESPONSIBILITIES:
1. Conducting and overseeing patient assessments (telephonically/in person) to aid patients in navigating and selecting the appropriate Urology care pathway. Navigator will assist in seeing that patient, who if undecided, are enabled to attend education/visit and information sessions regarding the program offerings in identifying the program or provider that would best meet their needs.
2. Where appropriate, provide a welcoming encounter (via phone or in person) for the patient
that reflects an ease of use into one of the Urology or Urogynecology locations and ease of getting timely access to education and information regarding the specialty's offerings.
3. Assessing patients clinically, and using information gathered, protocols, and offering recommendations to the patient about the care pathway that best suits the patient's needs.
4. Working with other staff, as well as support staff, provides guidance to patients through the care pathway that has been selected for them, in seeing that they are informed and are actively engaged in the program. Performing minor outpatient clinical duties as directed by the physician within the scope of their licensure.
5. Coordinating with Providers, outpatient departments, hospital departments, and other support staff in seeing that patients are engaged and compliant in the program and are well connected to those within each department.
6. Works collaboratively with all the Surgical Division programs to ensure that patient preferences and desires are maintained and supported and that the patient unique needs are addressed. This may include transitioning the patient, based on their preferences, and clinical information obtained through the patients experience to other program offerings that may be better suited to achieve the results the patient desires.
7. Participating in meetings and coordinating with various teams within the Surgical Division to contribute to the improvement of patient care, patient satisfaction, clinical outcomes that lend to the achievement of the department goals.
8. Remaining current with research and treatment options.
9. Participates in follow-up phone calls to assist patients with post-discharge instructions, follow up care, continuity of care, results and to keep the patient from readmission after surgery. Seeking patient feedback regarding program in opportunities to improve.
10. Serving as an educational resource for staff; assisting in developing multidisciplinary training programs.
11. Contributes to ongoing quality improvement plans and demonstrates this willingness to be participant.
12. Tracking patient outcomes, patient satisfaction and financial data to support the program, processes and improvement activities. Reporting key statistics as necessary.
13. Other duties as assigned.
REQUIRED SKILLS AND KNOWLEDGE:
1. In-depth knowledge of issues and treatment related to Surgical and other available options for patients based on clinical assessment (telephonically) to determine the best course of action for the patient.
2. Knowledge of resources within the community. Working knowledge of all office equipment including computer, keyboard, printer, fax, etc.
3. Excellent communication skills; computer literate. Ability to work efficiently in (EPIC) EMR system.
4. Managing a clinic schedule for first visit consults and follow-up appointments, performing physical exams, interpreting lab results, prescribing and managing medication, completing preoperative evaluations, and ensuring patients fulfill their insurance requirements for surgical services.
5. Communication barriers between patient/families, disciplines, departments and community agencies. Encourage collaboration and bring the interests of the patient and the collaborative team together.
6. Demonstrated ability to work successfully in a team-based decision-making culture.
7. Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
8. Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
9. Must respond and follow through to requests from customers promptly.
10. Must work carefully and precisely with attention to detail.
11. Must utilize resources wisely.
12. Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same.
13. Cooperates with other departments and work groups.
