Responsibilities
Position Summary:
The RN Hospice Case Manager delivers nursing care to patients in their homes and coordinates their treatment plans with a supportive, multidisciplinary team. This role blends hands-on care with case management responsibilities in a rewarding, patient-centered environment.
Key Responsibilities:

Provide direct, skilled nursing care to patients in the home setting
Conduct initial assessments and develop individualized care plans
Evaluate and revise plans as needed, collaborating with team members
Administer medications, perform wound care, IV therapy, and other clinical procedures
Educate patients and caregivers on treatments, medications, and disease processes
Ensure timely, accurate clinical documentation of care provided
Verify patient eligibility and compliance with home health guidelines

Qualifications:

Active RN license in the state of Massachusetts
Minimum 2 years of nursing experience
At least 1 year of home health experience preferred
Strong communication, assessment, and critical thinking skills
