RN Case Manager

The RN Case Manager is responsible for coordinating the continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Day Shift: 8AM-4:30PM (Monday-Friday)

Full time with benefits

Pay: $50-$55/hour
