Responsibilities include creating a care plan, monitoring patient medical necessity, advocating for resources, maintaining dialog with the care transition team, and promoting the spirit embodied with the Mission Statement.

Requirements include licensure as a Registered Nurse, education from an accredited school/college of nursing or professional licensure, and preferred credentials such as BLS Provider, and case manager credentials.
