The RN Care Manager will play a key role in facilitating communication, managing utilization of healthcare resources, and addressing social determinants of health to enhance patient outcomes.

Job specification

Coordinate patient-centered care through collaboration with interdisciplinary teams, ensuring smooth transitions across the continuum of care.
Perform comprehensive assessments to develop and implement individualized care plans.
Facilitate communication between patients, families, and healthcare providers to ensure effective care coordination.
Monitor patient progress, identify barriers to care , and make adjustments to treatment plans as needed.
Utilize evidence-based practices to support efficient resource utilization and optimize patient outcomes.
Educate patients and caregivers on disease management, treatment options, and self-care strategies.

Candidate requirements
Required:

Bachelor's Degree in Nursing (BSN) or equivalent education/experience .

Active California Registered Nurse License upon hire .

Minimum of two (2) years of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.) .

Minimum of two (2) years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Managers .

Preferred:

Master's degree in Nursing or a healthcare-related field .

Certification in Case Management (ACM or CCM) upon hire .
