This role is responsible for assessing, planning, coordinating, and advocating for patient care to ensure optimal health outcomes. As an RN Care Manager, you will collaborate with patients, families, and interdisciplinary healthcare teams to develop comprehensive care plans that align with best practices and cost-effective strategies.

Job specification

Perform comprehensive patient assessments to determine care needs and establish individualized care plans.
Facilitate care coordination, discharge planning, and resource allocation to support patient transitions across various levels of care.
Advocate for patient needs , ensuring access to appropriate services, treatments, and community resources.
Collaborate with physicians, nurses, social workers, and other healthcare professionals to deliver integrated and patient-centered care.
Monitor and evaluate patient progress, making adjustments to care plans as necessary.
Educate patients and families on disease management, treatment options, and self-care strategies.
Ensure compliance with healthcare regulations, policies, and best practices in case management.

Candidate requirements

Bachelor's Degree in Nursing or equivalent education/experience.

Active California Registered Nurse License upon hire.

Minimum 2 years of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.).

Minimum 2 years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Managers. TIP candidates must have experience in the same type of nursing unit in which the CM position is available.

Preferred

Master's Degree in Nursing or a healthcare-related field.

Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification upon hire.
