Schedule- 4-10 Hour Days, 12p-10p (may require an occasional weekend)

Responsibilities

• Fulfills the role of an RCC completing patient admissions.
• Serves as a resource to all other RCC's in completing admissions.
• Leads communication and coordination of care between all members of the care team.
• Assists all patients and care team members throughout the care model by acting as a patient advocate.
• Assists market manager in coordination of staffing and ensures appropriate resources are available to support patient care.
• Ensures that all new RCCs have the appropriate tools and templates to operate.
• Supports market manager in coordination of clinical and operational workflows and documents those steps.
• Assists market manager in maintaining data and reporting accuracy.
• Serves as a subject matter expert in identifying and solving negative trends and hurdles involved in health care system and incorporates solutions for system challenges.
• Builds and maintains collaborative professional working relationships with all care team members and is available to provide guidance and support during times of urgency or critical needs.
• Assists as needed in recruiting, interviewing and training of new team members.
• Performs other duties as assigned.

Qualifications

• Associate of Science in nursing.
• Current, active RN state licensure.
• Three to five years of acute nursing experience. Possess a general knowledge of clinical standards and outcome measurement.
• Reliable transportation and proof of liability insurance.
• Ability to support after-hours schedules and on-call needs.

Preferred

• Bachelor of Science in nursing.
• Three months of experience as an acute care RN providing in-home care.
• Two to three years of recovery care coordinator experience.
