Residential Case Manager - Louisville, KY
Full-Time | Direct Hire or Contract-to-Hire | $42,000-$45,000 + Retention Bonus
Are you passionate about making a tangible impact in the lives of individuals transitioning out of homelessness? A pioneering supportive housing facility in Louisville is seeking dedicated Residential Targeted Case Managers to join their growing team. This is a rare opportunity to be part of a groundbreaking initiative-Kentucky's first single-site supportive housing model-designed to provide holistic care and stability for high-need individuals.
Locations:
  • Primary Site: Southeast Louisville (onsite parking)
  • Secondary Site: Downtown Louisville - occasional meetings held here
Schedule:
  • Monday to Friday, 8:30 AM - 4:30 PM (36.25 hours/week considered full-time)
  • Flexibility required to accommodate client needs
  • Approximately 25% of time spent in the field supporting clients
Role Overview:
This position focuses on helping individuals secure and maintain permanent housing while providing wraparound support services. You'll work directly with clients-many of whom are transitioning from encampments-offering assistance with appointments, court dates, transportation, and daily living needs. The role is rooted in trauma-informed care, harm reduction, and strengths-based case management.
Key Responsibilities:
  • Conduct client assessments and develop individualized service plans
  • Facilitate monthly home visits and life skills group sessions
  • Coordinate referrals and accompany clients to services
  • Maintain relationships with landlords and act as liaison for housing stability
  • Provide crisis intervention and monitor service outcomes
  • Document activities in compliance with funding requirements
Ideal Candidate:
  • Bachelor's degree in a behavioral science; required
  • Has prior experience working with homeless populations and individuals with severe mental illness or behavioral health challenges; required
  • Brings case management experience and is willing to pursue Targeted Case Manager certification (agency-supported)
  • Possesses a valid driver's license, reliable transportation, and willingness to transport clients
Compensation & Perks:
  • Hourly Pay: $22-$24.20/hr depending on experience
  • Annual Salary: $42,000-$45,000
  • $2,000 retention bonus after one year

Pay Details: $42,000.00 to $45,000.00 per year
