Key Responsibilities:

• Provide comprehensive care for patients with acute and chronic illnesses, particularly among older adults.
• Take full accountability for patient care outcomes and engage with specialists for consultations as needed while remaining involved in patient care.
• Collaborate closely with hospitalists when caring for hospitalized patients.
• Manage assessment, diagnosis, treatment, care coordination, and documentation for patients with complex health needs.
• Lead a care team to promote patient health in-office and coordinate with transitional care teams for patients unable to visit.
• Contribute actively to center management, cover for absent providers, and assist in patient recruitment.
• Perform other duties as assigned.

• Availability for patients to establish trust and provide care as if for family members.
• Commitment to evidence-based medicine and understanding lifestyle factors that influence health outcomes.
• Strong physician leadership skills focused on developing your team and achieving quality patient care.
• Demonstrated desire for continuous improvement in medical practice and operational efficiency.
• Effective communication skills to influence patients and care teams compassionately.
• Proficient in Microsoft Office Suite and willing to travel as needed for training and recruitment.
• Fluency in English is required; knowledge of other languages may be beneficial.

Education and Experience:

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty required.
• Must obtain a State Medical License for the location of work.
• Board certification in relevant specialty preferred; Board eligibility is required.
• Basic Life Support (BLS) certification required.
• Must hold a current DEA number for controlled substances.
