Join a leading Managed Long Term Care (MLTC) organization as an RN Case Manager. We have multiple positions available for dedicated professionals who will primarily serve members in their assigned area. This hybrid role provides the flexibility of working remotely while requiring one field visit per week for in-home member assessments.

Key Responsibilities:

Manage a dedicated caseload, ensuring that members receive appropriate and accessible care.
Develop, oversee, and update individualized care plans to meet the evolving needs of members.
Conduct 4-6 home visits per week to assess home safety and care requirements.
Collaborate with leadership and Intensive Care Management teams to address complex cases.
Present high-risk cases for multidisciplinary review.

Qualifications:

Active New York State RN License is required.
Bachelor's degree in Nursing, Health, or Human Services is preferred.
A minimum of two years of home care experience is essential.
Strong expertise in care planning, patient assessments, and evaluations of home safety.
Prior supervisory experience is a plus.
Bilingual candidates are strongly preferred.
Proficiency in Microsoft Office is necessary.
