As a Regional Director for Case Management, you will play a vital role in enhancing care coordination programs, focusing on case management, social work, and care transitions. You will provide strategic guidance and leadership across our facilities, aiming to improve clinical outcomes and drive performance.

Key Responsibilities:

• Manage case management and social work teams, ensuring effective coordination across the care continuum.
• Provide strategic direction to cross-functional teams to successfully implement care transformation initiatives.
• Monitor adherence to care progression standards, maintaining communication with stakeholders to address progress and risks.
• Utilize data analytics to forecast needs and improve patient outcomes while reducing costs.
• Stay abreast of regulatory requirements and industry best practices to ensure compliance and innovation.
• Develop and implement innovative care management programs, focusing on efficiency and cost-effectiveness.
• Lead and develop your team, ensuring high levels of engagement and performance.
• Foster an environment that prioritizes patient care and operational excellence.

Education and Experience:

• Bachelor's Degree required.
• A minimum of seven years of experience, with at least five years in a leadership role.

Certification Requirements:

• Registered Nurse (RN) license in a Compact State or certification from the Missouri or Oklahoma nursing board.

This role is based in the Oklahoma City region and supports our three hospitals in that market, as well as our facility in Jefferson City, MO. Relocation assistance available as needed.

Work Schedule: Day Shift (40 hours per week)
