Job Summary: Under the supervision of the leadership team, manages patient referrals for program services recommended by their provider. This position will primarily focus on ensuring patient referrals are routed to the appropriate programs and disciplines.

Requirements:

• HS Diploma/GED preferred
• 1 to 3 years of related experience
