POSITION DESCRIPTION

POSITION TITLE: Reentry Case Manager

LOCATION/DEPT: Pathway Forward Program

REPORTS TO: Program Manager - Case Management

PAY GRADE: 205

PAY RANGE: $19.97-$23.96

OUTCOMES

Carries an assigned caseload of residents currently in the custody of the Federal Bureau of Prisons (B.O.P.) or the United States Probation and Pretrial Office. Provides comprehensive case management and other services targeted to enhance resident competencies related to a successful transition into the community. Works with each assigned resident to promote a productive lifestyle, reduce the risk of recidivism, and address specific factors that impact the individual resident's criminality and reentry. Oversees the resident's progress through the re-entry program from intake to release. Ensures that all case files are properly organized, up to date, and accurate. Contributes to promoting an overall rehabilitative environment at the re-entry center.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Intake

  1. Works with Intake Department and all other Pathway Forward Program staff to ensure that resident files are properly set up and all necessary intake and orientation procedures are completed on time.
  2. During initial meeting with newly arrived resident, ensures inquiry about need for special accommodation given documented health issues. Ensures inquiry about need or preference for special or restricted diet. After completion of 'Medical Screen' and verification of need for follow-up health care, prepares 'Request for Approval for Medical Treatment and Reimbursement' (Med. Cert), forwarding this in timely manner along with required documentation to B.O.P. Healthcare Services.
  3. Within 72 hours of a newly assigned resident's arrival, completes a 'Prison Rape Elimination Act' (P.R.E.A.) Initial Screening. Prior to assigned residents' completion of 30 days in program, administers 'Prison Rape Elimination Act' Re-Screening.
  4. Reviews the residents' financial status and processes Indigent Grant Fund requests for indigent residents to obtain medication. Arranges transportation for residents with serious medical conditions to report for medical appointments and/or pick up medication.
  5. As part of the intake process, undertakes a comprehensive assessment of the resident's strengths, needs, and risk factors for recidivism. Within 7 days of the resident's arrival, administers the 'Correctional Assessment and Intervention System' (C.A.I.S.).
  6. Upon residents' completion of required intake, forwards request for Program Review Team (P.R.T.) initial staffing. Prepares 'Program Staffing Report' in advance of initial staffing and presents case at initial P.R.T. staffing.
  7. Works with residents referred by the United States Probation and Pretrial Services. Ensures that each assigned resident requiring completion of physical exam within 5 business days complies with Intake Department and meets requirement. Reports all concerns to the assigned Probation Officer (U.S.P.O.).

Resident Interaction

  1. Meets with each assigned resident every 14 days, entering comprehensive 'Supervision Plan' at each meeting which reflects the resident's current accountability and re-entry progress (or lack thereof), including their progress in achieving goals established in the 'Preliminary Treatment Plan'.
  2. Ensures responsiveness to resident concerns.
  3. Enters required resident movements into the community according to program guidelines/parameters. Monitors resident movement in the community and ensures resident accountability.
  4. Ensures residents receive required legal documentation in timely manner. Provides residents with verification letters to assist I.D. update.
  5. Ensures assigned residents report for mandated aftercare.

Employment

  1. Communicates with the Learning Resource Center (L.R.C.) to ensure resident completion of requirements. Along with the L.R.C., assists the resident identifying employment, school, or job training opportunities appropriate to the resident's education and skill set.
  2. Maintains collaborative relationships with outside agencies. Facilitates referrals of residents, as deemed appropriate for that resident, to outside job readiness/job placement agencies, job training or certification programs, or school programs.
  3. Prepares request for employment spot check upon verification of the resident's job placement. Conducts weekly telephonic spot checks after resident starts employment to ensure resident's continued job attendance. Ensures conferral with assigned United States Probation Officer before permitting residents under court jurisdiction to resume or begin employment.
  4. Assists the resident in compliance with income reporting. Ensures verification of first pay date and pay cycle via conferral with the resident's employer.
  5. Assists the resident in formulating a budget based on verified monthly income and expenses.

Special Requests

  1. Addresses all special requests, unusual circumstances. Must also address certain emergencies immediately, as they arise. Sends special reports to B.O.P. for such occurrences.

Monitoring

  1. Monitors GPS tracking and monitoring requirement to ensure that residents are not deviating from their designated sites. When a malfunction occurs with a GPS unit, RCM refers the resident to the Intake Department for troubleshooting or replacement of the GPS units.

Case Files

  1. Maintains assigned case files that are complete, accurate, current and in readily auditable condition. Ensures case file materials are in specified sequence and neat, orderly appearance of case file. Responds in timely manner to audits

Home Confinement

  1. Reviews residents Home Confinement (H.C.) Eligibility and submits Home Confinement Review Forms for residents who have reached their Home Confinement Eligibility dates but have not been placed on H.C.
  2. Prepares the resident for 'Home Confinement' (H.C.) placement.

Relocation

  1. Processes residents' requests for out-of-district relocation.

Discipline Issues

  1. Ensures that residents abide by the rules and regulations of the Pathway Forward RRC. Issue "Disciplinary Incident Reports" and Informal Resolutions for residents not compliant with such rules and regulations.
  2. Issues "Serious Incident Reports" to notify key staff of a non-disciplinary incidents.

Release

  1. Prepares the residents for release from custody.
  2. Completes a comprehensive Terminal Report which includes a summary of the resident's program participation and the services he/she received. The Terminal Report also outlines the date of completion and outcome of such items.

Staffing

  1. Meets individually with Program Manager - Case Management for staffing of assigned cases. When necessary, attends monthly staffing of mental health aftercare to ensure case coordination with Clinical Services. Also attends monthly Reentry Case Manager and All Staff meetings.
  2. The Federal Bureau of Prisons reserves the right to change or modify the terms of its contract with the Salvation Army Pathway Forward Program. As such, the 'Job Responsibilities' outlined above may be changed or modified to accommodate for the new requirements.

Other Duties

  1. All other duties as assigned.

REPORTING RELATIONSHIPS

This position reports to the Program Manager - Case Management.

In contacts related to this position's duties this individual acts as a representative of The Salvation Army and its mission.

PERFORMANCE MEASUREMENTS

This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy, and completeness of performance.

A performance review will be conducted.

EDUCATION/EXPERIENCE

  • A bachelor's degree in a relevant discipline and either proficiency in a second language useful to program operations or exceptional work experience. MSW or related degree preferred.
  • Experience in case management.
  • Counseling experience strongly preferred.

POSITION LIMITATIONS

  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Program Manager - Case Management informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • This position is required to do light physical work and must be able to lift up to 20 pounds.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
  • The work environment for this position is a residential environment with a low to moderate noise level.
  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors. This would include but not be limited to such actions: resolution of conflicts in a positive manner; courteous treatment of staff, clients, and visitors; respect of others' property and person, professional and appropriate communication to and about co-workers, supervisors, and subordinates.
