About the Role

• Provide psychiatric evaluations and medication management to patients in skilled nursing and long-term care facilities
• Collaborate with an interdisciplinary care team to enhance patient outcomes
• Develop and implement individualized treatment plans tailored to each patient?s needs
• Build lasting relationships that improve both patient well-being and facility outcomes

Qualifications

• Active and unrestricted California nurse practitioner license (PMHNP certification required)
• Active and unrestricted DEA registration
• Minimum 6 months of psychiatric medication management experience
• Excellent clinical assessment, communication, and collaboration skills
• Genuine passion for geriatric and post-acute mental health care
