Compensation: $136,105 $163,105 annually (depending on experience) Sign-On Bonus: $4,000 (paid in three installments) Relocation Bonus: Up to $6,500

We are seeking a dedicated Nurse Practitioner to join a respected outpatient health center in the Santa Cruz area. This is a rewarding opportunity to provide high-quality primary care services to patients across all ages in a supportive, mission-driven environment.

Responsibilities:

• Provide comprehensive primary care services, including evaluation, diagnosis, and treatment
• Deliver preventive care and chronic disease management
• Collaborate with physicians and other providers to ensure quality patient outcomes
• Maintain accurate and timely documentation in the electronic health record
• Promote patient education and wellness

Qualifications:

• Licensed Nurse Practitioner in the state of California (or eligibility)
• Current DEA registration (or ability to obtain)
• Strong clinical and communication skills
• Passion for patient-centered, community-based healthcare
