Hiring Locations

We are actively hiring Physical Therapists (PTs) for home care opportunities in:

• Westchester County, NY

• Rockland County, NY

• Bronx County, NY

Key Responsibilities

• Assessment: Evaluate patients' functional limitations and develop personalized therapy plans with patients, families, nurses, and physicians.

• Direct Care: Deliver therapy services using accepted physical therapy modalities for medical and surgical conditions.

• Education: Train and supervise patients, families, and home health aides on exercise programs and care techniques.

• Ongoing Evaluation: Monitor and adjust therapy plans as patients' conditions evolve.

• Communication & Documentation: Report updates to physicians and managers, and maintain accurate clinical records of evaluations, treatments, and outcomes.

Qualifications

• Active New York State Physical Therapist (PT) license & registration (required)

• Minimum 1 year of full-time physical therapy experience

• Home care experience preferred (not required)

• Ability to use technology (tablets) for documentation; Oasis experience a plus

• Valid driver's license and reliable automobile required for field visits

Compensation & Benefits

• Salary: $95,000 - $108,000 annually (based on experience)
