Location: Allina Health Home Infusion Therapy Services Coon Rapids

Shift: Day (United States of America)

Shift Length: 8 hour shift

Hours Per Week: 40

Union Contract: Non-Union-NCT

Weekend Rotation: Every 5th

Job Summary: Guides patients to appropriate home infusion and home care services. Optimizes access to Allina programs and serves the Allina hospital and clinic partners by assessing patient referrals. Develops relationships with community referral sources.

Principle Responsibilities:

• Coordinates referrals on behalf of the patient
• Financial Vitality
• Proficient use of Electronic Health Record and other electronic tools for data gathering and communication to patients
• Exceptional Customer Service
• Other duties as assigned

Required Qualifications:

• Must be 18 years of age with education and/or experience needed to meet required functional competencies
• 0-2 years experience as a Pharmacy Technician

Preferred Qualifications:

• High school diploma or GED
• Vocational or Technical Training from an American Society of Health-System Pharmacists (ASHP) accredited pharmacy technician training program
• 2 to 5 years experience as a Pharmacy Technician with experience using data systems and/or direct patient care team support
• 2 to 5 years experience in customer service, care coordination, working with insurance, health care or community based role

Licenses/Certifications:

• Registered Pharmacy Technician - MN Board of Pharmacy required upon hire
• Certified Pharmacy Technician - Pharmacy Technician Certification Board required within 180 Days or
• Pharmacy Intern required

• Light Work:
• Lifting weight up to 20 lbs. occasionally, up to 10 lbs. frequently

Pay Range: $22.71 to $31.13 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
